Parallel and Perpendicular lines explained by Khan Academy
Here is a explanation of parallel and perpendicular lines and their slopes given by Khan Academy.  There is one example problem at the end.

Parallel and Perpendicular Lines – equations from a point

How to do the problems that ask for the equation of a line that is either parallel or perpendicular to a line and passes through a given point.
 
They use point-slope form and then convert it to slope-intercept form.  If you are asked to convert to Standard form, then use algebra to put the equation in Standard form.
